If you become a patient, care is provided by physicians licensed in your state of residence, using telehealth technology in accordance with applicable state and federal law. Before your first clinical encounter, you will be given a separate informed consent for telehealth and our full HIPAA Notice of Privacy Practices, both of which govern the clinical relationship. Telehealth has inherent limitations compared to an in-person visit; your physician will advise you if your condition requires in-person evaluation.
5. Quiz and Self-Assessment Tools
Our quiz is an educational, self-reported tool designed to help you understand where you may be starting from. It does not diagnose any condition and does not determine whether treatment is appropriate for you. Only a licensed physician, after reviewing your full health history, can make that determination. Results are not typical and individual results may vary.
6. Prescription Medications and Pharmacy Services
If a physician determines that a prescription is appropriate for you, it is filled through a dedicated 503A compounding pharmacy licensed to prepare patient-specific medications. Compounded medications are prepared under a valid prescription in accordance with federal and state pharmacy law; they are not independently reviewed by the FDA for safety or effectiveness in the same manner as manufactured, FDA-approved drug products. Your prescribing physician will discuss the risks, benefits, and alternatives relevant to your treatment.
7. Intellectual Property
All content, materials, quiz structure, text, graphics, and logos on this website are the intellectual property of Wellness Renewed, LLC or its licensors. You may not copy, reproduce, or distribute any part of this site without our express written consent.
8. Text Messaging (SMS) Program Terms
By providing your mobile phone number on our website (even if marked optional), you authorize Wellness Renewed, LLC to send you text messages related to your inquiry and our services.

These Terms of Use are governed by the laws of the State of Tennessee, without regard to its conflict of laws principles, except where superseded by federal law or the specific state law governing your telehealth care.
